We offer three great PPO options through Blue Cross and Blue Shield of Tennessee (BCBST) – the choice is yours! 

  • All plans have both network and non network coverage. 
  • No referrals needed
  • All plans have calendar year deductibles
  • Calendar year out of pocket maximums
  • Lifetime maximums of $2 million worth of  coverage

There are 3 levels of coverage:

  1. Team member only
  2. Team member + one
  3. Team member + family

Base Plan – this is an excellent offering for those who would pay less weekly and still have a basic 70/30 medical coverage.

Buy Up Plan – this is an excellent offering for those that don’t mind paying a little more each week to have lower co pays, lower calendar year deductibles and lower calendar year out of pocket maximums than the base plan. 

High Deductible – is a great offering for those who would rather pay less weekly and still have the comfort of having coverage when needed.  This plan has higher calendar year deductibles and calendar year out of pocket maximums than the other two offerings.

Prescription Drug Benefits are included in all three medical plan options – MEDCO is the administrator of the prescription coverage for both the base and buy up plans.  Caremark is the administrator of the prescription coverage for the high deductible plan.  This is a 3 tiered co-pay plan for generic, brand and non preferred drugs.  There is both a 30 day retail and 90 day mail in option.

Dental – We offer two great PPO options through Delta Dental of Tennessee – the choice is yours!

  • Both plans have both network and non network coverage. 
  • No referrals
  • Both  plans have calendar year deductibles
  • Both plans have annual per person maximums
  • Preventative care 100%
  • Basic services 80%
  • Major services 50%

There are 3 levels of coverage:

  1. Team member only
  2. Team member + one
  3. Team member + family

Base Plan – The annual per person maximum is $1000
Buy Up Plan – The annual per person maximum is $1500

Buy Up Plan only:

  • Orthodontia services (to age 24 – lifetime max $1500)
  • TMJ (lifetime max $1500)

Vision – Vision coverage is provided through EyeMed. You are offered both network and non network coverage
* Eye exams each plan year 100% paid after co-pay in network.

  • Eye glass frames once every 24 months
  • Lenses and contact lenses once each plan year

Life Insurance, AD&D Benefits and Short/Long Term Disability
This coverage provides financial protection for you and your family in the event of death or serious accident.  (administered by ING)

  • Basic Life – Management team members are provided and automatically enrolled in basic life/AD&D coverage for 1X your salary at no cost.
  • Supplemental Life – you have the opportunity to purchase supplemental life insurance for yourself and your dependents at additional cost.

Disability (Short Term and Long Term)
Management team members are provided with and automatically enrolled in short term and long term disability coverage at no cost.

Flexible Spending Accounts – two options administered by Discovery Benefits.

Retirement Planning - Merrill Lynch

  • 401(k) program – making it an easy way to save for the future
  • Company match of $.50 on each dollar up to 3% of your contribution
  • Vesting – 4 years of employment
  • Qualified rollovers are accepted

Tuition Reimbursement

  • $1000 per calendar year

Vacations - Your personal time away from work! All vacation is earned from anniversary date to the day prior to anniversary date and is available to be taken the following anniversary year. All vacation time must be taken, there is no "cashing out" of vacation. Vacation time not used before your anniversary date will be forfeited for that year.

  • 1 week after 6 months of employment
  • 2 weeks after 1 year
  • 2 weeks and 2 days after 4 years
  • 3 weeks and 2 days after 5 years
  • 3 weeks and 3 days after 6 years
  • 4 weeks after 10 years

Sick Days

  • All managers are eligible for up to 5 paid sick days per calendar year for personal illness.
